Indicaciones terapéuticas: Analgésico, antineurítico y antiinflamatorio. Lumbalgias, cervicalgias, braquialgias, radiculitis, neuropatías periféricas de diversa etiopatogenia; neuralgias faciales, neuralgia del trigémino, neuralgia intercostal, neuralgia herpética, neuropatía alcohólica, neurobion 5000 diabética, síndrome del conducto del carpo, fibromialgia, espondilitis. Contraindicaciones: Hipersensibilidad a los componentes de la fórmula.

Policitemia vera. La vitamina B 12 no debe ser utilizada en la enfermedad de Leber (atrofia hereditaria del nervio óptico). Úlcera acidopéptica gastroduodenal. En pacientes cuyos ataques de asma bronquial, urticaria o rinitis aguda son precipitados por el ácido acetilsalicílico o sus derivados. Restricciones de uso durante el embarazo y la lactancia: No se administre durante el embarazo y la lactancia.

Antes de administrar este medicamento, se deberá investigar el estado del aparato digestivo, del hígado y del riñón.

Precauciones generales: Diclofenaco: antecedentes de sangrado, úlcera o perforación gastrointestinal, insuficiencia renal, neurobion 5000 arterial no controlada o cardiopatías que cursen con retención hídrica y/o edema. Insuficiencia hepática, infecciones graves concomitantes, asma, porfiria, trastornos de la coagulación. Tiamina: antecedentes de alergia a las preparaciones que contiene tiamina.

Piridoxina: convulsiones neonatales, tratamiento simultáneo con levodopa. Cianocobalamina: el tratamiento con cianocobalamina puede enmascarar la deficiencia de ácido fólico. En grandes dosis puede corregir la megaloblastosis causada por la deficiencia de vitamina B 12, pero no previene las complicaciones neurológicas, las cuales pueden ser irreversibles.

Los pacientes con anemia perniciosa neurobion 5000 deficiencia de vitamina B 12 secundarias a un defecto irreversible de la neurobion 5000 requieren terapia con cianocobalamina de por vida. Puede presentarse una respuesta clínica adecuada a la cianocobalamina en presencia de infección, enfermedad renal, tumores o deficiencia concomitante de ácido fólico o hierro.

Reacciones secundarias y adversas: Se han publicado informes aislados de reacciones adversas por la administración parenteral a largo plazo de tiamina y vitamina B 12; lo que puede deberse, probablemente, a casos raros de neurobion 5000. La administración de megadosis de piridoxina puede producir ciertos síndromes neuropáticos sensoriales; sin embargo, los estudios histopatológicos no han demostrado que tales síndromes están relacionados con algún grado de degeneración neuronal.

Cuando se suspende la piridoxina mejora paulatinamente la disfunción neuronal, hasta que los pacientes se recuperan por completo. Erupción cutánea y otras reacciones neurobion 5000 hipersensibilidad conocida a cualquiera de los componentes de la fórmula.

Policitemia vera. Sistema gastrointestinal: dolor abdominal, náuseas, vómito, diarrea, dispepsia, flatulencia, anorexia; rara vez: hemorragia gastroduodenal, melena, hematemesis, ulceración, perforación, diarrea sanguinolenta. Ocasionalmente: Colitis ulcerativa o proctocolitis de Crohn, gingivoestomatitis, lesiones esofágicas, glositis, estreñimiento. Sistema nervioso central: vértigo, aturdimiento, cefalalgia, fatiga, rara vez: parestesias, trastornos de la sensibilidad y de la visión, trastornos de la memoria, desorientación, tinnitus, insomnio, irritaciones psicóticas, alteraciones del gusto.

Piel (casos aislados): erupciones vesiculares, eccema, eritema multiforme, síndrome de Stevens-Johnson, síndrome de Lyell, eritrodermia (dermatitis exfoliativa), alopecia, reacciones de fotosensibilidad, púrpura. Riñón (rara vez): hematuria, proteinuria, insuficiencia renal aguda.

Hígado (rara vez): elevación de la actividad de las aminotransferasas (transaminasas glutamicopirúvica y glutamicooxalacética), hepatitis con o sin ictericia. Sangre (casos aislados): trombocitopenia, leucopenia, anemia hemolítica, anemia aplásica, agranulocitosis.

Hipersensibilidad (rara vez): hipotensión arterial, edema, reacciones anafilácticas. Interacciones medicamentosas y de otro género: Se ha informado que la tiamina puede aumentar el efecto de agentes bloqueadores neuromusculares, desconociéndose su importancia clínica. El fosfato de piridoxal refuerza la descarboxilación periférica de levodopa y reduce su efectividad en el tratamiento de la enfermedad de Parkinson.

La administración concomitante de carbidopa con levodopa previene este efecto de la piridoxina. El clorhidrato de piridoxina no debe ser administrado en dosis superiores a 5 mg al día, a pacientes que reciben levodopa sola.

La administración de 200 mg al día, durante un mes, de clorhidrato de piridoxina produce disminución en las concentraciones séricas de fenobarbital y fenitoína hasta 50%.

La cicloserina y la hidralacina son antagonistas de la vitamina B 6 y la administración de la piridoxina reduce los efectos secundarios neuronales relacionados con el uso de estos compuestos. La utilización prolongada de penicilamina puede causar deficiencia de la vitamina B 6. Cuando se administra simultáneamente piridoxina y ciclosporina pueden disminuir las concentraciones plasmáticas de ésta última. La absorción de la vitamina B 12 en el aparato gastrointestinal puede reducirse por la administración de los fármacos siguientes: aminoglucósidos, colchicina, preparados a base de potasio de liberación prolongada, ácido aminosalicílico y sus sales, anticonvulsivantes (fenitoína, fenobarbital, primidona), irritación con cobalto en el intestino delgado y por ingesta excesiva de alcohol por más de 2 semanas.

La administración concomitante de neomicina y colchicina incrementa la mala absorción de la vitamina B 12. El ácido ascórbico puede destruir neurobion 5000 importantes de la vitamina B 12 y de factor intrínseco, en condiciones in vitro, por lo que esta posibilidad neurobion 5000 considerarse cuando se administren grandes dosis de ácido ascórbico de manera concomitante a la vitamina B 12 por vía oral.

Se ha informado que la prednisona incrementa la absorción de vitamina B 12 y la secreción del factor intrínseco, en algunos pacientes con anemia perniciosa, pero no en pacientes con gastrectomía parcial o total. Se desconoce la importancia clínica de estas observaciones. La administración concomitante neurobion 5000 cloramfenicol y vitamina B 12 puede antagonizar la respuesta hematopoyética a la vitamina. La administración simultánea de diclofenaco con preparados a base de litio o digoxina o con diuréticos ahorradores de potasio, puede elevar las concentraciones plasmáticas de estos fármacos.

Se recomienda practicar la farmacovigilancia correspondiente. La utilización concomitante con otros antiinflamatorios no esteroideos, pueden incrementar el riesgo de efectos secundarios adversos.

Se debe practicar vigilancia estrecha en pacientes tratados con anticoagulantes. Se deben suspender los antiinflamatorios no esteroideos 24 horas antes de administrar un tratamiento con metotrexato, para evitar que se eleve la concentración plasmática del citostático y se produzcan los efectos tóxicos del mismo. Precauciones en relación con efectos de carcinogénesis, mutagénesis, teratogénesis y sobre la fertilidad: No existen evidencias de efectos carcinogénicos, neurobion 5000, teratogénicos y sobre la fertilidad, en el ser humano y en los animales de experimentación.

Dosis y vía de administración: DOLO-NEUROBION ® Grageas: Oral. 3 grageas al día, preferentemente después de los alimentos. Se puede prolongar el tratamiento cuando el médico lo considere necesario.

DOLO-NEUROBION ® Solución inyectable: intramuscular profunda. Una ampolleta No. 1 y una ampolleta No. 2, mezcladas en la misma jeringa, una vez al día por dos días. DOLO-NEUROBION ® FORTE tabletas: Oral. 3 tabletas en el día, preferentemente después de los alimentos. Se puede prolongar el tratamiento cuanto el médico lo considere necesario. DOLO-NEUROBION ® FORTE Solución inyectable: intramuscular profunda.

Mezclar la ampolleta No. 1 con una ampolleta No. 2 en una jeringa estéril. Aplicar una vez al día. Manifestaciones y manejo de la sobredosificación o ingesta accidental: No se han descrito casos de sobredosificación por tiamina o por vitamina B 12. La neuropatía sensorial y los otros síndromes neuropáticos producidos por la administración de megadosis de piridoxina mejora paulatinamente al descontinuar la vitamina, y al cabo de un tiempo prolongado se obtiene la recuperación completa.

En caso de intoxicación aguda con diclofenaco se deben aplicar medidas de apoyo y sintomáticas. Se desconoce un cuadro específico. Las medidas a tomar son aplicar medidas de apoyo para: hipotensión arterial, insuficiencia renal, convulsiones, irritación gastrointestinal y depresión respiratoria.
